Job Title: Support Worker (Community Care Workers)
Service: Snodland, Lenham, Maidstone Kent
Rate: £24,000 annual salary (for age 21+, to be reviewed after Probation)

We are seeking passionate and dedicated individuals to join our team as Community Care Workers. Community Care is about contributing to the well-being of individuals who prefer to receive care in the familiar and comfortable surroundings of their own homes. This type of care is designed to help people, often those who are elderly, disabled, or chronically ill, to maintain their independence and quality of life while receiving the assistance they need with daily activities.
Key Responsibilities:
Provide personal care and assistance with daily living activities in accordance with individual care plans.
Community Care services can encompass a wide range of activities, including assistance with personal care (such as bathing, dressing, and grooming), medication management, meal preparation, light housekeeping, companionship, and support with mobility.
Establish and maintain effective communication with clients, families, and other members of the healthcare team.
Monitor and report changes in clients' conditions to the appropriate healthcare professionals.
Maintain accurate and up-to-date client records.
Qualifications and Skills:
Previous experience in a similar role is desirable, but not mandatory.
Compassionate and empathetic attitude towards clients.
Strong communication and interpersonal skills.
Ability to work independently and as part of a team.
Reliable and punctual with a strong sense of responsibility.
Willingness to undergo relevant training and development.
